If you want to register online and pay by debit card, credit card, or bank account ACH, please use this form. You can complete the form online.  A convenience fee of 3.25%, not to exceed $7.00, will be added to your invoice for payment by debit or credit card only. There is no charge for payment by bank account ACH.
